Physical Form

All PAISD athletes entering the 7th – 12th grade must have a Pre-Participation Physical Examination annually. As stated by the UIL, NO OTHER Physical Exam Form can be accepted. You can access this form by visiting PAISD's Rank One Sports site. You may fill out the medical history form online and then print it if you choose to do so. This form must be completed and signed by both the student and parent. The physical form must be completed and signed by a PHYSICIAN and must be dated after May 1 of the corresponding year. Both forms must be turned into the athletic trainer or the student’s coach ASAP.
